Parents’ Daily COVID Attestation Form
**Updated March 1, 2021: The enrollment process for the 2021-2022 school year has not yet begun. Please sign up for the SED All Parents email list (scroll to the bottom of this page to do so) as that is how the enrollment process will be communicated.**
As of March 1, 2021, SED is aware of the possibility that the public schools will resume in person education soon. We are working with SPS to understand the process and implications of this change to the learning model, and we will communicate with SED families as we have information about subsequent changes to SED’s program model.
The Before and After School Program for the families of Sudbury, Massachusetts
The Mission of Sudbury Extended Day is to provide the children of Sudbury’s working parents with nurturing before and after school programs that offer diverse opportunities for enrichment, exploration, and the development of social skills.
Sudbury Extended Day is an independent, not-for-profit school-age childcare program offering both before and after school care.
In a typical year, SED offers daily programs for children in grades Kindergarten through 8th grade, early release Wednesday coverage, and school holiday and vacation week programs. The program follows the Sudbury Public Schools calendar. Our programs are located in the four elementary schools, the Curtis Middle School, and the First Parish Church of Sudbury (Kindergarten only).
SED programs focus on the social and emotional development of our students within the context of such activities as crafts, nature studies, art, reading, science, sports, field trips, free play and games. The opportunity to play outdoors is vital to our children’s healthy development and is encouraged daily weather permitting.
In 2020, we are offering before and after school at the elementary schools only, and a Remote Learning Center between the hours of 7:30 to 2:30 at the First Parish Church. Program days are Monday, Tuesday, Thursday and Friday. This year there is no SED program on Wednesday, nor are we currently able to offer any programs on holiday or vacation days.
If you are already registered with SED (application, deposit and contract have all been received by the office), please complete the Online Health Forms as your final step for enrollment. These online forms are NOT to be considered as enrollment forms for our program. Thanks!
